Bibliography

Bibliography

This bibliography consists of citations for every publication cited in the main text of the book. Each citation includes references to the section or sections where the work is cited. Thus, this bibliography can serve as in index to discussions related to particular authors, organizations and their publications.

Citations follow the U.S.A. national standard ANSI Z39.29-1977 American National Standard for bibliographic references (American National Standards Institute 1977). A revision of this standard was approved in 2003. The only significant change for our purposes was moving the placement of dates for periodical articles from the end of the citation, after volume and issue numbering and pagination, to prior to the volume and issue numbering (National Information Standards Organization 2004?). We did not adopt this small change.
